BAGUIO CITY — Patients struggling to pay hospital bills may still seek government assistance through the Department of Health’s Medical Assistance to Indigent and Financially Incapacitated Patients (MAIFIP) program—but the Department of Health-Cordillera Administrative Region (DOH-CAR) is stressing that not everyone automatically qualifies, and assistance is not a blank check.
The clarification comes as MAIFIP continues to provide financial support to patients facing medical expenses they cannot afford. DOH-CAR said eligibility depends on an assessment of the patient’s financial circumstances, the medical need, applicable deductions, and the availability of program funds.
Who can receive MAIFIP assistance?
According to DOH-CAR Malasakit Program Unit head Kurthlyn C. Pablo-Paulino, the program serves two major groups.
The first is indigent patients—those with no visible means of income or insufficient resources to meet their basic needs.
The second is financially incapacitated patients. These are people who may have an income but, because of their present financial situation, cannot afford the medical treatment they need.
In both cases, qualification is not automatic. A hospital medical social worker or designated officer must assess and certify the applicant’s eligibility.
This distinction is important because MAIFIP is designed to respond to a patient’s actual financial incapacity rather than simply provide unrestricted assistance to anyone with a medical bill.
MAIFIP does not simply pay the entire hospital bill
One of the most important points in the latest DOH-CAR clarification is how the assistance is calculated.
MAIFIP is intended to help reduce the patient’s remaining out-of-pocket medical expenses after applicable hospital discounts, PhilHealth benefits, private insurance and other available funding sources have been applied.
Any remaining balance may be considered for MAIFIP assistance, but approval remains subject to program rules, social-worker assessment and the availability of funds.
The national MAIFIP guidelines issued through DOH Administrative Order No. 2026-0031 provide the framework for eligibility, documentary requirements, availment procedures, service coverage, charging arrangements and fund authorization for the 2026 program.
What medical expenses can MAIFIP cover?
The program’s coverage is broader than hospital confinement alone.
Previous DOH explanations of MAIFIP have identified assistance for FDA-approved medicines, laboratory and diagnostic procedures, blood products, medically indicated procedures, dental services, dialysis beyond applicable PhilHealth coverage, post-hospitalization and rehabilitation services, hospital charges after applicable deductions, and professional fees—subject to the program’s rules and requirements.
This means patients should not assume that every medical expense will automatically be approved. The particular treatment, documentation, eligibility assessment and available funding all matter.
Private-hospital patients may also qualify
Another significant development is that MAIFIP assistance is no longer limited to patients treated in government hospitals.
DOH-CAR said patients may receive assistance in participating private hospitals, particularly when the required medical service is unavailable in a government facility, provided the applicable agreements and MAIFIP rules are met.
In the Cordillera, DOH-CAR identified four private facilities participating in the program:
- Pines City Doctors Hospital
- Saint Louis University–Sacred Heart Medical Center
- Notre Dame de Chartres Hospital
- Cordillera Hospital of the Divine Grace in La Trinidad, Benguet
Patients are advised to verify the participating hospital and available services with DOH-CAR or the facility before applying because coverage and availability can vary.
What documents should applicants prepare?
DOH-CAR said applicants or their authorized representatives may initially prepare a valid government-issued ID, along with a Barangay Certificate of Indigency or Certificate of No Income, when applicable. A 4Ps identification card may also be presented when applicable.
Depending on the assistance being requested, applicants may additionally be asked for documents such as a hospital bill or statement of account, prescription, or diagnostic request.
Incomplete documentation can delay processing.
For patients who cannot provide the usual proof of indigency, DOH-CAR said they may request an eligibility assessment from a registered social worker at the hospital or through the City Social Welfare and Development Office.
A major warning: No double claims
DOH-CAR is also reminding beneficiaries about a rule that could easily be overlooked.
Patients cannot claim MAIFIP assistance and assistance from another government agency for the same medical expense.
The restriction is intended to prevent duplicate claims and make sure limited public funds reach more qualified patients.
In other words, patients should disclose other forms of assistance already received or being processed for the same expense rather than attempting to collect multiple government payments for one bill.
₱75 million already released in Cordillera
The scale of the program in the region is significant.
As of July 29, 2026, DOH-CAR reported releasing approximately ₱75 million in MAIFIP assistance to 1,330 patients from Abra, Baguio City, Benguet, Kalinga and Mountain Province.
That is substantially higher than the 453 patients assisted in 2025, showing the growing demand for medical financial assistance in the Cordillera.
The latest regional figures come as the national government continues to refine the program’s implementation.
In January, the Philippine News Agency reported that DOH was preparing new MAIFIP guidelines, which were subsequently formalized through Administrative Order No. 2026-0031 dated February 25, 2026.
Meanwhile, GMA News previously reported that MAIFIP was allocated ₱51 billion for 2026, after Congress increased the proposed funding during deliberations on the national budget. The program had received ₱41.16 billion in 2025.
What patients should remember
For families facing a medical crisis, the most important message from DOH-CAR is simple: MAIFIP can provide a crucial financial lifeline, but eligibility and coverage must be established through the proper process.
Patients should approach the hospital’s medical social welfare unit, prepare the necessary documents and disclose other assistance received for the same medical expense.
Most importantly, patients should not assume that having a hospital bill automatically guarantees MAIFIP assistance.
The program is intended for those who are indigent or financially incapacitated, and every application remains subject to assessment, documentation, applicable deductions and available government funds.
For Cordillera families facing overwhelming medical costs, knowing these rules before filing an application could make the difference between a delayed request and a properly processed one.
