PUERTO PRINCESA CITY, Palawan — Health authorities are urging workers who have spent time in malaria-prone areas, particularly forests and other known mosquito-breeding sites, to undergo blood smear testing as health officials intensify surveillance amid a continuing rise in malaria infections in Palawan.
The warning comes after the Puerto Princesa City Epidemiology and Surveillance Unit (CESU) and Pilipinas Shell Foundation, Inc. observed that many malaria patients had either traveled to or worked in areas where malaria transmission is known to occur.
The Philippine Information Agency reported on August 13 that health authorities specifically advised people with exposure to malaria-infested areas to have their blood tested so infections can be detected early and treated promptly.
Palawan remains the center of the country’s malaria problem
The concern is particularly significant in Palawan, which has remained the Philippines’ principal malaria hotspot even as most of the country has moved toward malaria elimination.
Earlier reporting from the Department of Health noted that 80 of the country’s 81 provinces had been classified as malaria-free, leaving Palawan as the lone province still dealing with ongoing transmission.
The latest 2026 figures show why health officials remain on alert.
As of June 20, 2026, MIMAROPA had recorded 1,343 malaria cases, up from 1,108 during the same period in 2025 — an increase of roughly 21 percent.
Palawan accounted for 1,308 of those infections, or about 97 percent of the regional total. Rizal municipality was the biggest hotspot, with 956 cases, followed by Bataraza with 125, Brooke’s Point with 96 and Quezon with 90. Puerto Princesa recorded 35 cases during the same period. No malaria-related deaths had been reported in the region as of that reporting period.
That concentration makes occupational and travel exposure particularly important for disease surveillance.
Military personnel among those affected
In Puerto Princesa, Barangay Tiniguiban recorded the highest number of cases in the city in 2026, according to the PIA report.
The area is home to the 3rd Marine Brigade, and health authorities said military personnel contracted malaria while participating in exercises in mountainous areas.
Of Puerto Princesa’s 53 reported cases, 30 involved people with a travel history while 23 were considered endemic or locally acquired cases, according to the city surveillance unit. Tiniguiban accounted for 11 cases.
Health officials also identified Iwahig as a common destination among patients with travel histories. Blood smear testing was conducted on approximately 1,800 people in the area, the PIA reported.
Other areas identified in connection with malaria cases include Bacungan, Montible, Napsan, Santa Lucia, Macarascas, San Rafael, Cabayugan and Inagawan Sub in Puerto Princesa, as well as Balabac, Aborlan and Rizal.
Why workers in forests face a higher risk
Malaria is transmitted primarily through the bite of infected female Anopheles mosquitoes. People who spend extended periods outdoors in transmission areas — including forest workers, soldiers, agricultural workers, travelers and other personnel assigned to remote locations — can face increased exposure.
The disease can initially resemble other common febrile illnesses, making early recognition difficult.
The World Health Organization warns that malaria symptoms can begin mildly, but untreated Plasmodium falciparum malaria can progress rapidly into severe disease and can become fatal.
That is why testing is critical when someone develops fever or other compatible symptoms after visiting a malaria transmission area.
A blood test can help detect the parasite
A blood smear is one of the established laboratory methods used to identify malaria parasites in blood.
Microscopic examination can help determine whether parasites are present and, depending on the diagnostic process, identify the malaria species involved.
For people who have recently worked or traveled in a malaria-risk area, health authorities’ message is straightforward: don’t wait for symptoms to become severe before seeking medical evaluation.
The warning is especially relevant because malaria infections can initially produce nonspecific symptoms such as fever, chills, headache and weakness.
The bigger challenge: stopping transmission
The latest increase in cases highlights the difficulty of eliminating malaria from Palawan.
The Philippines has made substantial progress in reducing malaria transmission nationwide, but Palawan continues to face unique challenges because of its geography, forested areas and continuing movement of people into and out of transmission zones.
Research on malaria in the Philippines has likewise identified Palawan as the country’s major transmission hotspot, with Plasmodium falciparum continuing to account for a significant share of infections.
The current situation therefore goes beyond individual testing.
Early diagnosis, prompt treatment, mosquito-control measures, use of protective clothing and insecticide-treated nets, and surveillance of people entering or leaving high-risk areas all remain important components of malaria control.
What workers should remember
Anyone who regularly works or travels in a known malaria-risk area should take precautions against mosquito bites and seek medical attention if fever or other malaria-compatible symptoms develop.
Workers should tell healthcare providers about their recent travel or occupational exposure, particularly if they have spent time in forests or other known malaria transmission areas.
And for people specifically advised by local health authorities to undergo testing, getting a blood smear can help determine whether malaria parasites are present and whether treatment is needed.
The warning behind the numbers
Palawan’s malaria problem is not simply about a rising case count.
The more immediate concern is where infections are occurring and who is being exposed.
With more than 1,300 infections already reported in Palawan during the first half of 2026, health officials are effectively warning that people entering forests and other transmission zones should not assume that being young, physically fit or previously healthy makes them immune.
For workers whose jobs take them into malaria-prone areas, the next fever should not be dismissed as just another ordinary illness.
Early testing could make the difference between catching an infection while it is manageable and allowing a potentially dangerous disease to progress.
