Scientists Say Your Real Age May Not Be on Your Birth Certificate — This Little-Known Measure Could Reveal How Well You’re Actually Aging

For years, the booming longevity industry has promised increasingly sophisticated ways to tell people how well — or badly — they are aging.

There are biological-age tests, DNA methylation “clocks,” blood biomarkers, fitness scores and wearable devices monitoring everything from sleep to heart-rate variability.

But scientists are paying growing attention to a much simpler question:

What can your body and mind actually do?

That question sits at the heart of a concept called “intrinsic capacity,” a World Health Organization-backed framework that researchers believe could become an important way of measuring healthy aging.

Rather than simply counting diseases or birthdays, intrinsic capacity looks at a person’s overall physical and mental abilities — including how well they move, think, see, hear and cope psychologically.

And researchers are now investigating whether tracking those abilities could tell doctors far more about aging than chronological age alone.

What exactly is “intrinsic capacity”?

The World Health Organization describes intrinsic capacity as the combined physical and mental capacities a person can draw upon at any given time.

Researchers generally organize those capacities into five broad areas:

Cognition — including memory, attention and thinking ability.

Locomotion — strength, balance, walking and general mobility.

Sensory capacity — particularly vision and hearing.

Psychological capacity — including emotional and mental well-being.

Vitality — a broader category linked to energy, nutrition and the body’s ability to maintain physiological resilience.

The concept was incorporated into the WHO’s healthy-aging framework in 2015.

Importantly, WHO distinguishes intrinsic capacity from functional ability. A person’s ability to live independently is influenced not only by their physical and mental capacity but also by their environment — including housing, social support, transportation and access to healthcare.

That means two people of the same age — even two people diagnosed with similar diseases — can function very differently.

And that is precisely why scientists are interested.

Why scientists are looking beyond your chronological age

A birthday tells researchers how many years someone has been alive.

It does not necessarily reveal how well that person’s body is functioning.

A healthy and active 75-year-old, for example, could have far greater mobility, cognitive ability and independence than someone considerably younger who has experienced significant physical or neurological decline.

Intrinsic capacity is designed to capture some of those differences.

Recent research has strengthened interest in the idea.

A 2026 study involving 159,840 adults aged 50 and older across four large cohort studies found that intrinsic capacity was significantly associated with mortality and healthy longevity. Researchers concluded that it showed potential as a predictor of healthy longevity, although further validation and standardization remain important.

A separate systematic review and meta-analysis of longitudinal studies also found that higher intrinsic capacity was associated with lower risks of functional decline and mortality among older adults.

However, the researchers stressed an important limitation: different studies have measured intrinsic capacity in different ways, meaning scientists still need a more standardized system.

And that leads to one of the biggest developments in longevity science right now.

The US government is spending millions to turn the idea into a usable score

The Advanced Research Projects Agency for Health, or ARPA-H, has launched a major program called PROSPR — PROactive Solutions for Prolonging Resilience.

Its goal is ambitious: identify early signs of age-related health decline and develop methods for testing treatments designed to extend the number of years people remain healthy.

One Stanford-led project, called THRIVE, received an award worth up to $34.5 million to help develop what researchers call the PROSPR Intrinsic Capacity score.

The team plans to combine large existing health datasets and test whether the resulting score can accurately track changes during a one-year lifestyle intervention.

Researchers also want to develop technology capable of assessing elements of intrinsic capacity at home.

Why does that matter?

Because testing a conventional “longevity drug” presents an enormous problem.

To prove conclusively that a drug helps humans live longer, scientists could theoretically have to follow trial participants for decades.

ARPA-H hopes biomarkers and functional measurements linked to long-term outcomes could eventually allow researchers to evaluate potential healthspan therapies over much shorter periods — perhaps one to three years rather than decades.

That does not mean scientists have already created a validated test capable of predicting anyone’s lifespan.

They haven’t.

The current work is aimed at establishing whether such measurements can become reliable clinical endpoints.

The idea is already moving beyond laboratories

Another sign that intrinsic capacity is gaining attention can be found in New Mexico.

In July 2026, the University of New Mexico Health Sciences Center and the New Mexico Health Care Authority launched Vive Bien/Live Well, based on WHO’s Integrated Care for Older People approach.

The bilingual program allows residents aged 65 and older to perform health check-ins covering areas including memory, mobility, nutrition, emotional well-being, vision and hearing.

Participants can track changes and receive recommendations that can also be discussed with healthcare professionals.

Program organizers specifically caution that the app is not a diagnostic tool and does not replace medical advice.

The broader idea is prevention: detect small losses of capacity before they develop into severe disability or loss of independence.

Can you improve your intrinsic capacity?

That may ultimately be one of the most important questions.

Intrinsic capacity is not necessarily fixed.

WHO says it can be built, maintained, restored or lost over time and can be affected by disease, injury, aging, health behaviors, healthcare access and broader social conditions.

Researchers are therefore interested not only in someone’s score at one moment but in their trajectory.

Is their mobility declining?

Is cognition stable?

Is strength improving?

Has hearing loss begun affecting daily functioning?

A 2025 longitudinal study using data from the Longitudinal Aging Study Amsterdam found that older adults followed different intrinsic-capacity trajectories, with poorer trajectories linked to greater functional limitations and poorer quality of life.

In other words, the direction of change could eventually be as important as the number itself.

But don’t throw away your medical tests

Intrinsic capacity is not intended to make blood pressure, cholesterol, blood glucose, cancer screening or other established medical assessments obsolete.

And it should not be confused with many commercial “biological age” tests already marketed directly to consumers.

The scientific challenge is still substantial.

Researchers do not yet agree on one universal way to calculate intrinsic capacity. Different studies have used different cognitive tests, mobility assessments, nutrition measurements and sensory evaluations.

That makes comparing scores difficult.

A major 2026 review of the concept argued that standardized measurement tools will be necessary before intrinsic capacity can reach its full potential in clinical medicine and longevity research.

So for now, intrinsic capacity should be viewed less like a definitive “age meter” and more like an emerging framework for answering a question medicine has historically struggled to quantify:

How much physical and mental capacity does a person still have — and how quickly is it changing?

If researchers succeed in transforming that concept into a reliable, standardized measurement, the implications could extend far beyond telling people whether they are aging well.

It could give doctors a way to detect decline earlier.

It could give researchers a faster method for determining whether longevity interventions actually work.

And it could shift the focus of aging medicine away from simply asking how many diseases a person has.

The bigger question may instead become:

How much of your ability to live the life you want can you preserve as the years pass?

That could ultimately prove far more meaningful than the number of candles on your birthday cake.
